Perth Home Care is a small local registered NDIS provider, founded and operated by the experienced registered nurse who has a passion for supporting people with disabilities. We currently offer services to people with disabilities in Perth metropolitan areas. We aim to support participants in their life's journey ensuring their independence is always a top priority while ensuring all their social needs, domestic needs and living arrangements are catered to.

Perth Home Care has a team of experienced Support Workers, Enrolled Nurses, and Registered Nurse to support our clients. Our staffs hold the required qualifications and are highly trained to ensure quality and safety services. Our client's individualised care plans are developed and reviewed regularly by our experienced Registered Nurse. We compliant with NDIS standard of practices, and have internal and external audit done regularly. Our clients may have a peace of mind that they are working with a high quality team!
Person-Centred Care
We understand that not one care plan fits all. We will take the time to get to know you and develop an individualised care plan that fits your specific needs. This Person-Centred Care approach will enable Perth Home Care to support clients in ways that will increase their success at living as independently as they are able and allow clients to contribute to community life to the extent they choose to.
What is NDIS
NDIS (The National Disability Insurance Scheme) provides funding to eligible people with disability to connect them to services in their community. The services that funded by NDIS may includes daily personal activities, transport to enable participation in community, therapeutic supports, and help with household tasks.
There are three options to manage your NDIS funding;
self-managed, plan-managed and NDIA-managed.
